The clearest sign is a panel that cannot be added to. If every slot is occupied, or the panel is a fuse box, then any new circuit (a kitchen island, a bathroom, a charger, a heat pump) has nowhere to land, and no amount of rearranging changes that.
The subtler signs are about behavior rather than capacity. Lights that dim briefly when a large motor starts, breakers that trip under loads they used to carry, or a panel cover that is warm to the touch all point at a service working closer to its limit than it should. Warmth in particular is worth acting on: it means resistance somewhere in the panel, and resistance in a connection carrying household current is how a panel fire starts.
The rest are circumstantial and just as telling. Extension cords and power strips in permanent use are a house telling you it has too few circuits. A service change is permitted and inspected work, and the utility disconnect and reconnect are coordinated around that. The panel is rarely replaced on its own: the meter socket, the service entrance conductors and the grounding electrode system are usually part of the same job, because a new panel fed by old conductors and bonded to an incomplete ground is only a partial upgrade.
Inside the panel, each existing branch circuit is identified and landed on an appropriately sized breaker.
